To access the significant procedure fee discounts offered by this plan, we must adhere to the following payment policies:
- The one-year membership fee must be paid in full upon enrollment (no partial year refunds).
- The membership will be valid for one year from the date the annual fee is paid.
- This plan cannot be used in conjunction with dental insurance.
- Advanced Family & Cosmetic Dentistry reserves the right to decline treatment or terminate membership without notice if the member's account becomes delinquent at any time. All other terms and conditions of service at Advanced Family & Cosmetic Dentistry remain applicable.
Exclusions
The procedure fee discounts provided by this plan do not extend to treatment already underway prior to enrollment and cannot be used alongside dental insurance benefits, other discounts, promotions, or external financing plans (such as Care Credit or Chase Health Advance). Please note that this is not a dental insurance plan but rather a discount membership plan, and benefits are solely applicable to treatment offered by Advanced Family & Cosmetic Dentistry.
